Instructions for Downloading Forms

Copies of all schedules and forms are also available upon request. Reference copies of certain records retention and disposition schedules are available in Adobe Acrobat format (*.pdf). The reader may be downloaded from the following website: http://www.adobe.com/prodindex/acrobat/readstep.html

To download these forms:

  1. Right-click once on the link for the form you need. A small menu will appear.
  2. Select "Save Target As ... " (Internet Explorer) or "Save Link As ... " (Netscape Navigator).
  3. Select a directory on your hard drive (c:, for example) to save the file.
  4. Click the Save button.

To use these forms in Microsoft Word for Windows (v6.0 or later) for the first time:

  1. Select the name of the form you need from the list above.
  2. Either allow MS Word to start, or save the document, and conduct a virus scan.
  3. Open the document.
  4. In the box "Unprotect Document," choose cancel.
  5. Select the first field (blank).
  6. Enter the information requested.
  7. Move from one field to the next using the tab key. Use shift-tab to move backwards.

Local Records Forms (Counties and Municipalities)

Records management services are offered to all North Carolina counties and municipalities. Services include creation of general records retention and disposition schedules; on-site visitations by Local Records staff; and consultation assistance on the full range of records creation, maintenance, storage, and disposition activities and responsibilities. Subject to availability of adequate resources, security microfilming of permanently valuable records is provided at a minimal fee to local government agencies.
